You can easily add a Qualtrics survey link to a module, providing access through an external URL module item. Alternately, you can embed a Qualtrics survey into a page in Canvas.
Click on the survey that you would like to share in Canvas. At top, click on the Survey tab.
Beneath the Survey tab, click the Survey options icon.
Under the Options heading, click Security.
Make sure Available to anyone is selected. Qualtrics will save your selection automatically.
Click the Publish button at top right to publish your survey.
At the top of the survey, click the Distributions tab.
On the left, click Anonymous Link.
Copy the link.
Open a new browser window and log in to your Canvas course.
Click on Modules in your course menu.
In the module where you would like to embed the survey, click the + to add an item.
In the pop up, select External URL from the dropdown menu.
Paste the URL copied in (9) above into the URL box.
Add a name for the survey.
Click Add Item.
Publish the link: click the "no entry" icon to the right of the module name; you will then see green check mark icons, confirming that both the survey and the module have been published.
Students can now click on the link in the module to load the survey within Canvas.
Embed a Survey on a Page

In your Canvas course, create a new Page. Edit your page in HTML mode by selecting the </> under the Rich Content editor.
Once you are in HTML editor mode, the toolbar above the text entry box will disappear. Paste the following code into the text entry box:
