While watching the following video about the logistics of planning a home birth, identify the spots where you hear the provider discuss the culture of dialogue that supports effective and efficient transfers of care.

In this video the communication tools used in the procedures for a home birth transfer are described. Identify which tool is being listed at each time code provided. Then explain the difference in information that would be conveyed for each receiving provider.
