We have created our own shared decision making process to highlight the key elements in a person-centered decision making process (PCDM). Our process includes the interprofessional team and the third person as part of the process. The focus is on the person. The provider individualizes the care by eliciting the values and beliefs from the person receiving care and by including people the client may identify as important to involve in decision making process. Our PCDM process will be elaborated on at various points throughout the course. Take a really good look at it now to become familiar with the content. When you see our graphic again, we will highlight end explain the aspect of PCDM that we want to apply to the case. Look for units titled "A PCDM Key Element".
