3.4.g Keep the Focus on the Person: A PCDM Key Element

One role of the Decision Coach is to assess the person's preference for the amount and type of information and help them to interpret and understand their options and decisions. The Decision Coach acts as a facilitator during the PCDM process. They will help other providers to present the options and help clients to interpret the pros and cons of their options. After the choices have been presented and discussed the Decision Coach will clarify that the person understands their options and ensures that all client questions have been responded to by the team members.

The Decision Coach keeps the focus on the person. They individualize the care plan by helping the client to interpret their options within the client's own values, beliefs and preferences, and they also help clients to understand how the limitations or standards of the environment interact with the client's individual preferences.
